The first stage involves damage exclusively to organ tissue. There are A, B, C stages:

  • with B – malignant cells are located in both glands, other characteristics coincide with 1A;

  • at stage B, there is spread not only to the uterus and tubes, but also to other organs and walls located next to the tumor, but the washout test for cancer cells from the cavity is negative;

The third stage, which is classified into A, B, C forms, is characterized by the presence of cells in the peritoneum outside the pelvic area or metastases in the nearby lymph nodes. Two organs or one are affected. Differentiation of stages:

  • A – confirmed metastases by biopsy in the peritoneum outside the pelvis;
  • B – visible metastases in the peritoneum outside the small pelvis with a diameter of up to 20 mm;
  • C – metastases in the peritoneum outside the pelvic area more than 20 mm and/or spread of cancer cells to the hypogastric, iliac, lateral sacral, para-aortic, inguinal lymph nodes.

The last stage is described by the presence of distant metastases. If cancer cells are present in the liver capsule, stage 3 is diagnosed, with metastases in the parenchyma - stage 4. Also, stage 4 is diagnosed in the presence of a tumor in the pleural cavity.

Tissue varieties are represented by a variety of types and this is due to the presence of several types of cells. The tumor can come from:

  • epithelium (80%);
  • stroma (connective tissue, blood vessels) – 10%;
  • germ cells (10%).

Epithelial ovarian cancer forms the majority of malignant neoplasms of this organ and is diagnosed in women from 40 to 60 years of age.

Carcinomas that are formed from epithelial tissue include serous, mucinous, endometrioid, clear cell, squamous cell, Brenner tumor, and undifferentiated carcinoma.

Malignant cystomas or adenocarcinomas are secondary tumors, as they arose on the basis of benign ones. Serous ovarian carcinoma leads in the structure of epithelial tumors.

Cancer against the background of benign formations – cysts – develops with varying frequencies. Thus, serous adenomas become malignant in 15% of cases, mucinous adenomas in 5%.

Serous ovarian cancer responds well to chemotherapy compared to clear cell and mucinous cancer, however, not all patients are diagnosed at stages 1-2, which causes a high mortality rate. The higher the degree of differentiation, the better the prognosis in treatment.

  1. Serous swellings.   2. Mucinous tumors. Benign - adenofibroma, cystadenoma, cystadenofibroma; borderline - the same, but with signs of a low degree of malignancy; malignant - ovarian cystadenocarcinoma, cystadenofibroma, adenocarcinoma, adenofibroma.
  3. Endometrioid tumors. Benign - cystadenoma, adenoma, cystadenofibroma, adenofibroma; borderline - the same, but with signs of a low degree of malignancy; malignant - carcinoma, stromal sarcoma, mesodermal tumors.
  4. Clear cell tumors.
  5. Brenner tumors.
  6. Epithelial ovarian cancer and other mixed epithelial neoplasms.

Primary malignant tumors include:

  • Dysgerminomas are formations that form from rudimentary ovarian tissues and are characterized by a high rate of malignancy;

In this case, the concentration of CA-125 is determined once every three months for the first 2 years, during the third year once every 4 months, then once every six months for another 2 years.
